Consent for First Aid and Treatment

Accidents, injuries and emergencies occasionally occur during the course of the school day. In order to provide the best possible care for your child, district nurses will administer first aid and treatment, as prescribed by the school doctor in the School District's Standard Operating Procedures.

First aid and treatment may require the administration of the following but not limited to: topical anti-itch, antiseptic, antibiotic creams or gels, topical pain reliever, petroleum jelly, eye wash, saline solution, wax for braces, aloe gel, salt water gargle, lotion, heat or ice, hygiene and oral care products, providing a change of clothing, neo-synephrine, or epinephrine.  Epinephrine may be administered by a trained faculty/staff member.  For outdoor activities, a nurse, faculty/staff, or the student (when appropriate) may apply sunscreen to prevent sunburn. Sunscreen should be provided by the parent/guardian.

If you DO NOT wish to have school nursing professionals administer First Aid treatment to your child as described above, you may choose to opt out.

If you choose to opt out of your child receiving First Aid Treatment, please contact the nurse’s office in the building(s) your child attends. A form will be provided for your signature to opt out.
