Forms And Publications
Below you will find Elizabethtown Area School District's "Sports Registration Packet" and the Handbook for Student-Athletes and Parents of Student-Athletes. The registration packet contains the following four district forms:
  • Acknowledgement of Risk Form
  • Athletic Responsibility Acknowledgement Form
  • Parent Code of Conduct
  • Drug and Alcohol Acknowledgement Form
In addition, the packet contains a PIAA athletic physical form. All five forms must be completed and submitted to the athletic office prior to a child's participation on an Elizabethtown Area School District athletic team.
